Happens sometimes during event missions when farming. Although i think the best one was phalanx 2 on DMM - i use the thief so the mobs are stronger. Placed out 2 witches and the prince, then accidentally let my mind wander off and didn't wake up until i heard the prince dying. Managed to salvage it though.

If I have been using specific teams for farming something (say male team) then forget to switch to my main team for taking it on. My male team can beat the male only mission on H but no healers means it will likely die horribly to most other things. Similar things happen if I use my ranged only team on the wrong map by mistake.

yes. Sometimes monsters spawn differently a bit. For example , i think charlotte's event (if i'm not confusing her event with another) had a map like that were a skeleton could spawn earlier a bit and fuck up your entire map , because your duelist would block him instead of the main boss (a lich)
