To get a 2nd centy now without spending rainbow medals you'd have to craft it. You can get 60 small centies from 6(00) flower gems every 2 weeks, ~40 so far from the other shops, and a few others from giveaways and stuff. You need 100 so it's not too hard to have 2 right now.
To use them you just feed it to a girl. As mentioned before it can only be used on maxed bloomed rainbows.
