It can drop from the Advanced stage, yes. However, I definitely found it easier to pick up on the Ultimate stage..

I have completed the Ultimate stage 2 times, and you are not even certain to get the Special Item... the second time I got a random 2* Knight...

I couldn't try yet (not enough Sta), but what is under the Locked Panel "2" on the Top path (just after the random Arrow). An hidden Chest?

What kind of team speed/where are you guys placing for farming the Ultimate mission? My guys ran around the middle for a bit, but no one ever hit the boss on the bottom, and eventually one of the teams got out of the loop and finished the map.

Well it's just RNG, I used my standard Team there:

My Main and the Helper on the bottom camp (so 50% to success on that path), 1 squad in the middle (100%) and 2 on top (75%). The two times I played the map they killed all enemies.

Oh ok, I thought there was some special trick with the locks I had to do.

So we always just get the 1 chest only? And there's no point doing this once we have the new girl at max skill level right? On another note a lot of these event bosses seem to hit really hard but be made of paper, at least compared to the other bosses in the game.

I don't think so, I think the blossoms just count as having Charged Laser and to my knowledge there isn't another unit with that skill, but I could be wrong.

Gosh, had to do the Blue Dragon Quest the whole day and then failed to 2 Medal the Second Mission. I'm off to a great start and way behind you guys. :/
On a brighter Side Note for the Future i have now 80 Girls in Total and my Team can consist only of 4* and above.
