I don't know if I am doing this wrong but lately I have not been able to see the bonus from the 100% affection (the scene?). I once got a girl to 100% affection and nothing happened, I though a bonus was supposed to happen.
Am I doing anything wrong?

I have only played the version of the game that does not have any h-scenes, but apparently there is a version that has the scenes:

So check whether right before the final slash it has the "-online" part in the url. If it does, remove that to get the hentai version with bonus scenes.

Also know that while playing the .net version you can re-watch the H-scene's as much as you like.
